To best serve you, we will ask for your medical history and any physical or neurological examinations you have already had. This helps us not only diagnose you faster, but prevent you from having to take the same tests over again when they did not help you in the first place.
During your first visit, we want to get to know you. We want to know what level you are as an athlete, what you have been going through, and how you feel your pain is affecting you. We will go over your medical history during this appointment as well. If you have had any CAT scans, X-rays, or MRI scans done, we will review those too. If you have information from any of the following, please provide it during your appointment:
Pain Journal: a self written journal that you have used to keep track of your pain, symptoms, pain intensity, and triggers
- Medications: a list of any medications you are currently taking or have taken
- Treatments: an outline of the pain treatments you have tried or are currently trying
- Details: a list of what you have noticed makes your pain better or worse
